ACCEPTANCE LETTER
Congratulations and welcome to the Thunder Soccer Fest, August 30-31, 2008
 
Registration
 
Registration will be held on Friday, August 29th from 5:00 p.m. to 8:00 p.m., at the Discovery Sports Center located on the grounds of the Maryland SoccerPlex, 18031Central Park Circle, Boyds, MD 20841. For driving directions, please log onto www.thundersoccerfest.com.
 
1. Player Passes: All players must be registered with one (1) club. All teams (with the exception of U8 teams) must be affiliated with an appropriate USSF affiliate member – USYS State Association or US Club Soccer. It is required that each team have verification by an Association or properly validated State Registration form, of each player’s age and team membership. Verification must be presented to the Tournament Credentials Committee at Registration on Friday, August 29th. Failure to do so will result in the team being excluded from participation in the tournament.
 
Two (2) copies of each team’s validated Roster must be presented at Registration.  Player Passes will be checked against the validated roster at registration.
 
2. Loan Players: Teams requesting loan players may have a maximum of three loan players. Loan player requests must be received no later than August 28th. Forms are available on the website.
 
3.    Request for Travel Forms: Teams requiring request for travel must present their form at Registration.  
 
4.    Medical Release Forms: It is required that each team’s coach has in his/her possession, a medical release form for each player. The appropriate parent or guardian, authorizing emergency medical treatment must sign these forms. Your league medical release forms are acceptableIt is NOT required that the forms be notarized.  Do NOT send these forms – bring to registration.
5.  Maryland SoccerPlex Waiver of Liability:  Please insure that a form is completed for each of your players and presented at registration.  This form is a requirement of the SoccerPlex - players without the waiver form will be prohibited from participating in the tournament.  Forms are available on the website.
 
A team representative is REQUIRED to attend Registration.
 
Tournament T-Shirts
 
Available on the Thunder Soccer Fest website – www.thundersoccerfest.com - is a T-Shirt Pre-Order Form. If your team would like to take advantage of the pre-sale opportunity, please fill out the form and send, with accompanying check or money order, no later than August 22nd.  
 
Rules of Competition
 
Please read and print out the Rules of Competition posted on the Thunder Soccer Fest website prior to attending the tournament!
 
TEAM WITHDRAWAL
 
Should you decide to withdraw your team on or after receipt of this letter, you will forfeit your entry fee.. We hope you will be considerate of the time and effort that went into the team selection process.
